This chapter describes the potential far‐reaching consequences of contemplative higher education for the fields of science and medicine.

In 2018-2019, at the Keck School of Medicine of the University of Southern California, we developed and piloted a narrative-based health systems science intervention for patients living with HIV and medical students in which medical students co-wrote patients’ life narratives for inclusion in the electronic health record. The pilot study aimed to assess the acceptability of the “life narrative protocol” from multiple stakeholder positions and characterize participants’ experiences of the clinical and pedagogical implications of the LNP. Students were recruited from (...) KSOM. Patients and staff were recruited from the Maternal, Child, and Adolescent/Adult Center for Infectious Disease and Virology at Los Angeles County+USC Medical Center. Ten patients, seventeen students, and ten MCA staff participated in the pilot study. Qualitative methods were used to gather data from students’, patients’, and staff’s perspectives. Three themes emerged from the thematic analysis: patients’ life narratives conveyed their unique life experiences and voices; the protocol could result in wide-ranging effects on HIV care; the LNP enabled students to contribute value to patients’ healthcare. Across groups, participants considered the LNP an acceptable intervention. The LNP, its limitations, and implications for HIV care, narrative medicine, and health information technology are presented. (shrink)

The unified theory of dose and effect, as indicated by the median-effect equation for single and multiple entities and for the first and higher order kinetic/dynamic, has been established by T.C. Chou and it is based on the physical/chemical principle of the massaction law (J. Theor. Biol. 59: 253-276, 1976 (質量作用中效定理) and Pharmacological Rev. 58: 621-681, 2006) (普世中效指數定理). The theory was developed by the principle of mathematical induction and deduction (數學演繹歸納法). Rearrangements of the median-effect equation lead to Michaelis-Menten, Hill, (...) Scatchard, and Henderson-Hasselbalch equations. The “median” serves as the universal reference point and the “common link” for the relationship of all entities and is also the “harmonic mean” of kinetic dissociation constants. Over 300 mechanism-specific equations have been derived and published using the mathematical induction-deduction process. These equations can be deduced into several general equations, including the median-mediated whole/part equation, combination index theorem, isobologram equation, and polygonogram. It is proven that “dose” and “effect” are interchangeable, thus, “substance” and “function” are interchangeable, which leads to “the unity theory” (劑效、心物、知行一元論) in quantitative mathematical philosophy (數學的定量哲學) in functional context. Therefore, a general theory centered on the “median” and based on equilibrium dynamics has evolved. In other words: [「中」的宇宙觀： 以「中」爲基凖的動力學生態平衡]. Based on the median-effect equation of the mass-action law, the fundamental claim is that we can draw “a specific cure” for only two data points, if they are determined accurately. This claim has far reaching consequences since it defies the general held belief that two points can dray only a straight line. Remarkably, the unity theory (一元論) providesscientific/mathematical interpretation in equations and in graphics of Chinese ancient philosophy, including Fu-Si Ba Gua (伏羲八卦), Dao’s Harmony (和諧), the Confucian doctrine of the mean (儒家中庸之道), Chou Dun-Yi’s (周敦頤, 1017-1073) From Wu-ji to Tai-ji and Taiji Tu Sho (無極而太極及太極圖說). The moderntopological analysis for trinity yields an exact correspondence to the Ba-Gua, which was introduced over 4,000 years ago. Furthermore, the median-centered algorithm, promotes modern ecological content (生態學) in the equilibral dynamic state of harmony. It is concluded that Western science and Eastern philosophy are directly linked and complementary to each other. Since the truth in mathematical quantitative philosophy (數學的定量哲學) has no boundaries, East and West philosophies can flourish together for the common goal and ideal in science and in humanity (世界大同). Imagine that we are considering whether our healthcare system (or insurer) should fund treatment or procedure X. One factor that may be cited is that of so-called ‘medical necessity’. The claim would be that treatment X should be eligible for funding if it is medically necessary, but ineligible if this does not apply. Similarly, (and relevant to the debates in this special issue), if considering whether a particular treatment should be ethically and/or legally permitted, we may wish to distinguish between (...) cases where the treatment is medically necessary, and those were it is not. But what do we mean by this concept? Here I will propose and briefly defend one plausible and practical definition. (shrink)

In this paper I consider a recent argument of Timothy Williamson’s that epistemic internalism and content externalism are indeed incompatible, and since he takes content externalism to be above reproach, so much the worse for epistemic internalism. However, I argue that epistemic internalism, properly understood, remains substantially unaffected no matter which view of content turns out to be correct. What is key to the New Evil Genius thought experiment is that, given everything of which the inhabitants are consciously aware, the (...) two worlds are subjectively indistinguishable for them, which is what matters on internalist accounts of epistemic justification. I argue that even if a standard moral of the New Evil Genius intuition is untenable due to considerations arising from content externalism, the case can be understood as supporting epistemic internalism in a way that is wholly compatible with content externalism. In short, epistemic internalism is committed to sameness of justificatory status between subjectively indistinguishable counterparts, not sameness of content of their justifiers. (shrink)

Arising from a graduate course taught to math and engineering students, this text provides a systematic grounding in the theory of Hamiltonian systems, as well as introducing the theory of integrals and reduction. A number of other topics are covered too.

Infants who are born extremely prematurely can survive if they receive intensive medical treatment. However, they also have a high chance of dying, and a proportion of survivors have long-term health problems and disabilities. In many parts of the world, if parents request it, an extremely premature infant can receive palliative care rather than active survival-focused care at birth. But there are variations between countries as to whether or when this is permitted. To help inform ethical debates across Asia and (...) more widely, we present two contrasting views about parental discretion and the treatment of extremely preterm infants. In questions of this nature, disagreement and dissensus are inevitable. Differences in the outcomes of treatment, the resources available, and the values of society mean that we should not expect a uniform approach. We identify points of potential consensus and compromise despite disagreement. (shrink)

Can metaphysics yield the consolations of philosophy? One possibility, defended by Derek Parfit, is that reflection on the nature of identity and time could diminish both fear of death and grief. In this paper, I assess the prospect of such consolation, focussing especially on attempts to console a grieving third party. A shift to a reductionist view of personal identity might mean that death is less threatening. However, there is some evidence to suggest that such a shift does not necessarily (...) translate into less death anxiety. Moreover, applied to grief at loss of another, such a perspective may be misdirected. A temporally neutral perspective offers a theoretically powerful way of reducing the sense of loss at being separated in time from a loved one. However, it is unclear whether it is psychologically possible to achieve. Even if it were possible, it may not diminish the pain of separation.I identify a serious challenge to philosophical consolation for grief. The greater the consolation that is offered, the greater the risk of losing important attachments and the less it may be psychologically accessible. (shrink)

We first prove that every connected component of X is either a singleton or a generalised crown (i.e. an ordered set that is connected, has length 1, and all vertices of which have the same degree). The representation of a generalised crown by a square (0,1)-matrix in which all line sums are equal is used throughout, and a complete description of X, including the number (...) of connected components and the degree of the vertices, is given. We then examine the converse problem of when a generalised crown can be made into a connected component of (X; g). We also determine the number of non-isomorphic finite simple Ockham algebras that belong properly to a given subvariety P 2n,0. Finally, we show that the number of fixed points of (L; f) is 0,1, or 2 according to the nature of X. (shrink)

Individuals are generally held to be morally and legally responsible only for actions carried out freely and deliberately, that is to say, for actions that result from our free choice. However, there is a quite widespread view that all of our actions are the result of the scientific laws that govern our physical bodies. If this should prove to be the case, then human choice would be an illusion, and therefore -- on the generally accepted principle just stated -- personal (...) responsibility would be a meaningless concept and blame and punishment would be inappropriate responses to wicked or criminal acts. In this paper I shall argue that we need to disentangle ‘responsibility’ from ‘choice'. I shall also outline reasons for holding that responsibility is ultimately a first-person matter -- I am responsible if I feel responsible, irrespective of choice -- and I shall briefly consider some of the consequences of this approach.
